Life Team

The LIFE Team (Living Independently for Everyone) is an integrated health and social care team providing a range of short term support to people to individuals to retain or regain their independence at times of change and transition. This can include support to facilitate hospital discharge (Discharge to Assess Pathways) or reduce the risk of hospital admission.
