How to Access the Service (Occupational Therapy)

Scope of this chapter

The ACOT service works to enable people with disabilities to carry out essential activities, with the aim of maintaining or improving independence. The service can provide a range of aids and adaptations depending on the assessed need.

Internal referrals are referrals made by one of the three ACOT teams to another of the ACOT teams.

These referrals are made using the internal referral form.

All other referrals should be made following the external referral process.

External referrals are those being made by:

  1. A social work team or other adult social care service;
  2. A health professional or service;
  3. A person or carer (self referral).

All external referrals should be made via Croydon Adult Support.
